have at least 5 questions, not including screening and demographic questions.
You may have screening questions to be sure the respondent meets the criteria for inclusion in your study. Demographic questions include age, race, gender, sexual orientation, etc. You will need to include those if it relates to your topic. Remember you are NOT going to implement this survey.
