Develop a plan of action (Ultimate goal, intermediate goals, and steps to reach goals)
Provide a Health Diary for 8 weeks with at least two (2) detailed entries per week. Make sure journal entries includes reflection and your behavior.
After completing your Health Diary, write a paper discussing the following:
What Health Change Theory fit the behavior in your Health Diary? Why? Was it appropriate?
Did you succeed in your goal attainment? Why or why not?
As future health professionals, what did you learn from this process? How will it help you be a better professional?
