5-year-old female with past medical history of asthma is being seen in the Emergency Department with bilateral wheezing, subcostal, intercostal and sub clavicular retractions, nasal flaring, and a Spo2 of 88% on room air. The patient is alert and oriented but gets very out of breath when trying to talk. Perfusion and capillary refill seem normal

Patient has had symptoms for the last 24 hours when there was a dramatic change of temperature in the area. Mom gave the patient 4 puffs of Albuterol with a spacer every 4 hours for the last 24 hours, but she has seen no improvement. She reports the patient takes Qvar 2 puffs BID with a spacer and there are not compliance issues. The patient triggers are change of temperature, cold/virus and pollen.

A chest X-ray reveals hyper-inflated lung fields with no concerns for pneumonia or cardiac disease. There are not allergies, the child ate some breakfast before coming to the ED – but not much, there are no rashes, signs of trauma or bleeding seen.
