This needs to be an intervention case study. There is an outline that is attached specifically for this case study and has to be followed 100% or the case study will not be accepted by the program. I have attached a passed intervention case study that was rejected. These are the reasons why the case study was rejected-Diagnosis-anxiety disorders have not been explored even though the referral was for symptoms of anxiety and depression. Formulation- A good understanding of the purpose of case formulation was not demonstrated. The case report very briefly identifies the P factors but this was not well integrated in a discussion to account for presenting problems. Assessment- No risk assessment was included. Results of the assessment have not been summarized, interpreted and intergrated into the overall report. The results must be included within the report and not as an appendix.
