This project is to show that African American who are diagnosed with diabetes mellitus type 2 have better outcome when participating in the propose education program the intervention section of this paper.
PICOT format
P Problems (This study will focus on the underserved population of adults African American patients, both men and women of at least 21 years of age who are diagnosed with diabetes mellitus type 2. Patients with uncontrolled A1C level are often at risk to develop diabetic related complications.
I Interventions (Education program) enroll all qualify patients diagnosed with diabetes mellitus type 2 in a diabetic education program to help better manage their blood sugar to lower A1C level. (Diabetic management program to include a nutritionist, diabetic nurse educator and a physical activity specialist). Participants will include both newly diagnosed patients and patients that are currently following treatment for DM2. Newly diagnosed patients will get automatically enroll into the study and patients with prior treatment plan for DM2 will be assessed and adjustment will be made to their treatment plans to mirror the same treatments listed in the study intervention. A1C level will be checked every 3 months to monitor progress.
C – Comparison or Control (compare the A1C level of the patients on the experimental group with the patients that are not part of the diabetic education program/ non-experimental group).
O outcomes (For patients with diagnosed DM2 who participated in the research program to maintain lower A1C levels).
T Time (3-6 months to monitor blood sugar level and A1C level to determine if there is a different outcome between the two groups).
