Write an INCIDENT REPORT based upon the assumption that you are the trucking companys traffic manager. The company has had four highway accidents within one week. Your reader is the company president, Frederick Fuller. Use the following facts and supply additional appropriate and relevant details where necessary and visuals. Create a realistic or borrow an existing organization identity. (Do not use me as the reader of your report!)
The trucking company operates in your state.
The four accidents occurred in different parts of the state and on different dates
(within the same week). [Specify the date and location for each accident.]
Each accident resulted in damage to the truck [specify the dollar amount of the
damage] and the cargo [specify the type of cargo and the dollar amount of the
damage].
Only one of the four accidents involved another (non-company) vehicle. [Provide
details of how this accident occurred; give make, model, and year of the
damaged car and its owners name.]
Only one of the accidents involved injury to a company driver. [Provide this
drivers name and detail the injuries].
The companys maintenance division traced all four accidents to faulty tires, all of the same brand. [Provide the brand name.] All tires were purchased at the same time and place. [Provide the place and date.]
All tires purchased from this vendor have been replaced. [Provide the cost of replacing all tires.
Acme Underwriters, your companys insurance company, has brought suit against the tire manufacturer to recover damages, including lost business, while the trucks were being repaired. [Specify the dollar amount of the lost business.
SHORT REPORT EVALUATION
Conventions of Short Report (30 points):
o That the introduction states the subject and purpose; summarizes any
conclusions and/or recommendations.
o That the body presents a detailed account of work reported.
o That the conclusion summarizes findings and indicates their significance.
o That the recommendations (if appropriate) discuss actions you believe should be
taken based upon the conclusions.
o That the report follows conventions for the type of report written.
Content and Organization (40 points):
o That all necessary information and appropriate costs are included and detailed in
an easy-to-understand manner (like a table or list)
o That reasons are included for any recommendations.
o That a logical and conventional method of organization is employed.
o That you present a straightforward course of action to follow (if appropriate).
o That the report is directed to a specific audience.
o That the tone is businesslike, confident, encouraging, and positive.
o That the point of view is appropriate and consistent.
o That you employ a precise vocabulary and concise wording.
Appearance (10 points):
o Use of white space
o Readability
o Overall attractiveness
Grammar & Mechanics (20 points): Correct use of grammar and mechanics
Bonus points will be awarded (up to 10) for the effective use of graphics.
NOTE:
Assignments that do not meet the above criteria may not be accepted for credit and
will have points deducted.
