In addition you will explore vascular access considerations and complications a patient receiving renal replacement therapy may experience..

Mr Green is a 58-year-old gentleman, who presents to his GP complaining of experiencing, breathlessness, nausea, fatigue, chronic headache and short term memory loss.
Mr Green lives with his wife in a medium sized country town. He has been married for 25 years and has three adult children. The children have relocated to the city for employment purposes. Mr Green has been enjoying life since retiring as a truck driver. Mr Green has a mild deformity of his non-dominant hand and this results in slight impairment of two of his fingers. He has no other relevant past medical history. Prior to commencing RRT, blood was taken for biochemistry and full blood count.

Mr Green is transferred to a large hospital under the care of a nephrologist.
A renal biopsy is performed and the diagnosis of end stage renal failure (ESRF) secondary to glomerular nephritis (GN) is made.
It has been decided that for the interim Mr Green will be commenced on HD.
Question 1 (approx 800-900 words)
Discuss your assessment and priorities of care for Mr Green prior to and during his first dialysis session. Consider within your discussion:
• evaluation and rationalisation of all pathology results
• rationale for current clinical symptoms (include but not exclusively pathology)
• essential requirements/ interventions prior to initial dialysis
• justification of dialysis modality for Mr Green at this time
• formulation and justification at a physiological level of an appropriate introductory dialysis regime for Mr Green. (this will require (not exclusively) some exploration of disequilibrium syndrome DES)

Question 2 (approx. 500 words)
It takes some time to effectively determine an ideal dry weight (IDW) for patients commencing on dialysis, explore the meaning of IDW and discuss the factors that will assist/ impact in determining an IDW for Mr Green.

Question 3 ( approx 1000 words)
It is possible that Mr Green may need to have a permanent vascular access created. Discuss the different forms of vascular access available for HD.
Ensure that you consider access/site selection/mapping, advantages, disadvantages and potential complications for each option, as well as risk reduction strategies relating to sustaining vascular access patency

Question 4 (approx 900 words)
Identify a minimum of 10 HD related complications a patient may experience whilst undergoing a treatment session and then discuss, the possible clinical manifestations, causes, immediate management and risk reduction strategies for four of the ten potential complications (this may not include D.E.S)

The policy Research Paper is a five unit-long project that allows you the opportunity to examine an issue of current
relevance to local or national public policy making. Policy papers typically have one main agenda: address a current issue
or policy and propose/evaluate alternative policies which seek to improve the current state.

Policy papers tend to be very methodical, as a similar process is used each time to evaluate the different policies you are
advocating to your client. While literature papers focus primarily on textual analysis and building one’s argument, policy
papers focus much more on organization, thoroughness, and execution. Basically, if your client only has 15 minutes to
read your policy memo during his or her commute from the office to city hall, you want to make it easy for him or her to
navigate through your paper.

The policy Research Paper should contain a comprehensive assessment of the problem, the proposed (or actual) policy,
and the potential (or actual) consequences of implementation of that policy. When you write your paper, you should think
of yourself as a staff person who must advise a policy maker facing a problem that must be resolved in the near future. As
this staff person, adopt one of the perspectives of public management as presented in this module, and frame your
responses around this approach.

The structure of public policy papers is different from typical papers and requires certain elements and sections in addition
to the traditional thesis, introduction, and conclusion.

When writing your public policy paper, make sure to include the following sections:

? Executive Summary: This goes at the very front of your paper and is a summary or abstract of your analysis. It is
absolutely essential that you include your policy recommendation here.
? Introduction: Explains to your audience why the issue/policy you have chosen to investigate is important and
relevant and what the current policy is. References to other research done on this issue can be included here as
well.
? Criteria: Let the reader know what criteria you are using to evaluate your policies. Economic feasibility, political
feasibility, and environmental impact are common criteria. Criteria should also be unique to your chosen
policy/issue.
? Alternatives: These are the policies you are analyzing and evaluating in your paper. Alternative policies seek to
improve on current policy and/or approach issues from different perspectives. These alternatives are evaluated on
the criteria you have listed out above.
? Projected Outcomes and Tradeoffs: This section can be included in the evaluation of each alternative policy or
set apart on its own. Much of your individual analysis and opinion falls under this section, as you are called upon
to not only predict the effects and efficiency of your policies, but also assess the negatives and positives of your
alternatives.
? Recommendation and Discussion: Here is where you finally decide among your alternatives and advocate your
policy. Remember not to simply repeat what you have covered under Alternatives and Projected Outcomes and
Tradeoffs, but explain why your client should follow your analysis and enact the policy you have chosen.

Your paper must:

? Be a minimum of 700 words;
? Be typed, double-spaced, using Times New Roman font (size 12), with one-inch margins on all sides; citations
and references must follow APA format; and
? Include a cover page containing the tile of the assignment (Public Policy Paper) the student’s name, and the
university name. The cover page and the reference page are not included in the required assignment page length.

Part I: Interview

Select a patient, a family member, or a friend to interview. Be sure to focus on the interviewee’s experience as a patient, regardless of whom you choose to interview.

Review The Joint Commission resource which provides some guidelines for creating spiritual assessment tools for evaluating the spiritual needs of patients. Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients.

Create a survey to assess the subject’s spiritual need during the interview. The spiritual needs assessment survey needs a minimum of five questions that can be answered during the interview. During the interview, document the interviewee’s responses.

Submit the transcript of the interview. The transcript should include the questions asked and the answers provided. Be sure record the responses during the interview by taking detailed notes. Omit specific names and other personal information from the interview.

Part II: Analysis

Write a 250-500 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:

What went well?
What would you do differently in the future?
Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges?
Describe the spiritual experience you had with your patient, family member, or friend using this tool. How does this tool allow you to better meet the needs of your patient?
Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? Explain your answer with examples.
Submit both the transcript of the interview and the analysis of your results.
